How this works (and how it’s different)

Most home health care services follow a similar pattern. When a senior’s medical or physical condition changes, they receive skilled intervention from a variety of clinicians upon their return home, with insurance covering the majority of the cost; services are then discontinued per insurance guidelines. Unfortunately, the health care system was built to respond to crises, not prevent them.

Healthy At Home works differently. We provide ongoing, scheduled wellness visits in your home. We monitor your health, update your exercise program, assess your environment for hazards, and coordinate with your medical team, not for six weeks until the authorization expires, but for as long as you need us.

This is a wellness practice, not a clinical rehab service. Katie’s 20+ years as a physical therapist inform everything she does, but what she’s offering with the wellness model is something insurance doesn’t cover, and the traditional system doesn’t provide: a sustained, relationship-based approach to keeping you safe, strong, and independent at home.

Every plan starts here: The Home Safety & Baseline Assessment

$250 (one-time)

Before recommending a plan, we need to see your home and understand your baseline. The assessment is a comprehensive, room-by-room evaluation that covers fall hazards, accessibility barriers, mobility and strength, medications, and daily routines. It typically takes 90 minutes to two hours.

  • Our recommendation for which wellness plan fits your situation. There is no sales pressure. If you don’t need ongoing services, she’ll tell you.

Monthly Wellness — $275/month

1 visit per month. Your early warning system.

Overview

Seniors who are largely independent but want a professional to keep watch. Katie visits once a month to check in on your health, review any medication changes, walk through your home for new hazards, and refresh your exercise program so it doesn’t go stale.

Best For

Active seniors who are doing well but want to stay ahead of problems, and families who want to know someone qualified is checking in regularly.

What a typical visit looks like

Katie visits once a month to take vitals, check in on how you’ve been feeling, and have able to perform day-to-day tasks and activities of daily living.

  • She observes you walking, checks your balance, and notes any changes.
  • She assesses your home for any potential new risks, such as changes in lighting, a new rug, or a new medication with side effects.
  • She updates your exercise program and leaves you with clear instructions.

The value

Catching the slow slide before it becomes a crisis. The medication change affects your balance. The slight limp you’ve been compensating for. The winter hazard you stopped noticing. These are the things that lead to falls, and falls are what end independent living.


Active Wellness — $450/month

2 visits per month. The bridge between rehab and real life.

Overview

Seniors managing a chronic but stable condition, such as Parkinson’s, heart failure, osteoarthritis, or anyone who’s finished a round of physical therapy and isn’t ready to go it alone. Active Wellness adds guided fitness sessions and consistent vitals monitoring to everything in Monthly Wellness.

Best For

Seniors who need more than a monthly check-in. If you’ve been discharged from PT but still feel unsteady, or you’re managing a condition that requires regular exercise and monitoring, this is where most clients land.

What a typical visit looks like

  • One visit per month follows the Monthly Wellness format, including a health check-in, home sweep, and program update.
  • The second visit is a 60-minute guided fitness session focused on strength, balance, and gait training tailored to your condition and home environment.
  • Katie monitors your vitals (blood pressure, oxygen saturation, blood glucose as applicable) at every visit and tracks trends over time.

The family connection

At the end of each month, Katie sends a written summary to your designated family member or Power of Attorney. What she’s seeing, what’s improved, what to watch for. For adult children managing a parent’s care from out of state, this is often the most valuable part of the entire plan and likely the first professional, honest update they’ve ever received.

The value

A typical outpatient PT copay runs $30–$75 per visit, 2–3 times per week, for 6–8 weeks. That’s $360–$1,800 out of pocket, and then it ends, whether you’re ready or not. Active Wellness costs $450/month and continues as long as you need it. No authorization. No discharge date. No starting over.


Complete Care — $975/month

4 visits per month (weekly). Comprehensive support at a fraction of facility cost.

Overview

For higher need seniors who require weekly professional support, or for motivated “super-agers” who want maximum investment in their independence, this is the package that fits the need. Complete Care includes everything in Active Wellness plus weekly visits, priority scheduling, and full medical team coordination.

Best For

Seniors with complex medical situations who need someone to quarterback their care. Or highly motivated clients who want the most comprehensive aging-in-place support available. And families who want total peace of mind by working with a professional they trust, in the home every week, with direct communication to them and to the medical team.

Care coordination — what it actually means

Katie becomes the central point of contact for your entire medical team. She calls the primary care physician with updates. She follows up with the neurologist after an appointment. She coordinates with the podiatrist, the cardiologist, and the home modification contractor. Nothing falls through the cracks because one person tracks it all, and that person also sees you in your home every week. This part of the offering is flexible and designed around the client’s goals and needs. Katie is there to support however the client and family feel this offering works best for their needs.

Families who’ve looked at assisted living and the cost averaging $8,635/month or more, and want to know if there’s a way to get that level of attention while staying home. There often is.

Katie will meet with you and your family, assess the situation, and build a plan with clear deliverables and pricing. No two Concierge plans look the same, because no two situations are the same.

Concierge is a custom plan designed around your specific needs. It includes everything in Complete Care, plus a visit schedule built for your situation, 24/7 phone access to Katie, and whatever level of coordination your medical team and family require.

No. Healthy At Home is a wellness practice, not a clinical rehab service. Katie is a licensed physical therapist with 20+ years of experience, and that expertise informs every visit — but these are wellness services focused on prevention and maintenance, not treatment of acute injuries or post-surgical rehab.

Insurance-based home health is episodic: a set number of authorized visits that end when you’re deemed “stable.” Private pay means no visit limits, no discharge dates, no rotating staff, and no documentation burden, all of which steal time from your actual care.
